About the role

PACE Senior Community Care of Jefferson County is part of Volunteers of America National Services, which serves as the Housing and Healthcare affiliate of the Volunteers of America parent organization. The position oversees both sites and conducts home visits in Jefferson County.

Responsibilities

• Oversees both sites and conducts home visits in Jefferson County
• Processes and completes admissions
• Provides pre-admissions counseling, information, and tours
• Reviews EPIC for proper documentation and orders
• Facilitates PAS Screening and Level II as needed
• Assists Social Services team with various tasks
• Participates in Vulnerable Adult investigations and completes necessary documents/reports

Requirements

• Bachelor's Degree in humanities/health and human services or equivalent experience with LPN certification
• Two (2) years of experience in marketing, with more emphasis on sales, and/or admissions role for a health care provider, or in an information and referral resource role within the senior networks
• Comfortable working in a fast-paced environment
• Excellent customer service skills

Qualifications

• Preferred qualifications include general knowledge of Medicare, Medicaid, insurance, VA, and skilled nursing experience

• Strong communication and interpersonal skills
• Ability to manage multiple tasks simultaneously
• Knowledge of healthcare regulations and policies

Pay

$58,000 - $65,000 (based on experience)

Schedule

M-F, 8a-5p, Occasional weekends
