Enhanced Community Coordinator

Responsibilities

  • Assessment and verification of service coordination needs
  • Service planning
  • Referral
  • Monitoring or service provision
  • Advocacy for consumers
  • Afterhours crisis services
  • Conducting onsite visits of service delivery sites for post-move monitoring
  • Reviewing Program Providers’ records and implementation plans
  • Intensive monitoring, including inquiries about recent hospitalization, ER visits, and crises
  • Documentation of progress
  • Participating in service planning team meetings
  • Frequent contact with family members and service providers
  • Submitting reports to HHSC as scheduled or upon request, including CLDP reports to SSLC and HHSC for individuals who transitioned from SSLC
  • Extensive travel within the local service area required

Requirements

  • Bachelor’s or advanced degree from an accredited college or university with a major in a qualified human services field to include-- but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice.
  • Or a high school diploma or GED and:
    • Two years of paid experience as a case manager in a state or federally funded Parent Case Management Program, or have graduated from Partners in Policy Making;
    • And have 2) personal experience as an immediate family member of an individual with intellectual and developmental disabilities.
  • Must have proficient computer skills to include MS Word, Excel and Outlook.
  • Must have valid driver’s license and an acceptable driving record, as well as personal automobile liability insurance as required by the state of Texas.

Qualifications

  • Two or more years of work experience as a Service Coordinator, case manager, or related position serving individuals with Intellectual and Developmental Disabilities
  • PREFERRED: Bilingual skills(English/Spanish)

  • Valid driver’s license and acceptable driving record
