Responsibilities
- Assessment and verification of service coordination needs
- Service planning
- Referral
- Monitoring or service provision
- Advocacy for consumers
- Afterhours crisis services
- Conducting onsite visits of service delivery sites for post-move monitoring
- Reviewing Program Providers’ records and implementation plans
- Intensive monitoring, including inquiries about recent hospitalization, ER visits, and crises
- Documentation of progress
- Participating in service planning team meetings
- Frequent contact with family members and service providers
- Submitting reports to HHSC as scheduled or upon request, including CLDP reports to SSLC and HHSC for individuals who transitioned from SSLC
- Extensive travel within the local service area required
Requirements
- Bachelor’s or advanced degree from an accredited college or university with a major in a qualified human services field to include-- but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice.
- Or a high school diploma or GED and:
- Two years of paid experience as a case manager in a state or federally funded Parent Case Management Program, or have graduated from Partners in Policy Making;
- And have 2) personal experience as an immediate family member of an individual with intellectual and developmental disabilities.
- Must have proficient computer skills to include MS Word, Excel and Outlook.
- Must have valid driver’s license and an acceptable driving record, as well as personal automobile liability insurance as required by the state of Texas.
Qualifications
- Two or more years of work experience as a Service Coordinator, case manager, or related position serving individuals with Intellectual and Developmental Disabilities
- PREFERRED: Bilingual skills(English/Spanish)
