What You’ll Do
- Ensure that referrals and initial outreach are documented accurately and timely.
- Maintain tracking and communication for program team members re: outreach success to support linkage with members.
- Identify care coordination needs/services in line with CANS outcomes and High-Fidelity Wraparound model certification to support transition to ongoing care coordination services and positive outcomes for children, youth and families.
- Complete utilization support tasks to ensure functionality and accuracy of EHR record and data as assigned.
- Participate in ongoing fidelity review and monitoring system focused on consistent application of system of care principles, adherence to ICC/MCC planning process and service components.
- Engage and build positive relationships with children, youth, and families in coordination with child and family teams to support successful integration of team members and care plans.
Qualifications
- You have a Bachelor's Degree human services field (preferred)
- You have experience related to children’s behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field, providing community-based services to children and youth, and their family or caregivers.
- Three years with a high school diploma or associate's degree
- Two years with a bachelor's degree (human services field)
- Master's degree (human services field)
