About the role
The Endocrine Navigator at Outer Cape Health Service (“OCHS”) reports to the consulting Endocrinologist and Director of Nursing. This role involves providing indirect patient care and support based on the nursing process within assigned work periods.
Responsibilities
- Make, review, and monitor appropriate referrals required for the management of Endocrine patient care.
- Outreach to patients to ensure proper follow-up appointments are in place, and to encourage follow-up with PCP and/or Endocrinologist as appropriate.
- Educate patients in the proper management of Endocrine disorders.
- Identify and maintain a current directory of outside sources, programs, or support groups, available for additional education or referrals based upon endocrine diagnoses.
- Refer patients to appropriate outside sources, programs, or support groups, for additional education or referrals based upon endocrine diagnoses.
- Maintain a current and accurate directory of local Endocrine practices and resources available to OCHS patients.
- Promote and support teamwork by collaborating with team members including providers, Medical Assistants, and Patient Service Representatives.
- Adhere to the National Patient Safety Goals that are applicable to ambulatory care centers, including, but not limited to, improving the accuracy of patient identification, improving the safety of using medications, reducing the risk of care-associated infections, and following the universal protocol of preventing wrong site/wrong procedure/wrong person).
- Perform other related duties and projects, as required or assigned in order to ensure the needs of the Health Center and its patients are met.
Qualifications
- Preferred: Current active Massachusetts Registered Nurse, Licensed Practical Nurse licensure; or Current active Massachusetts Dietician – Nutritionist License.
- Required: Minimum of 2 years’ experience in a health care field; preferably in dealing with Endocrine disorders; or 3 years’ other professional experience in Endocrine disorders.
- Maintain and enhance professional skills and knowledge.
- Excellent interpersonal skills, communication skills, organizational abilities, computer experience, and the ability to work within a collaborative team setting.
- Excellent written and verbal communication skills – bi-lingual helpful.
- Proficiency in computer skills including EPIC, Outlook, Microsoft office, OneDrive etc.
- Excellent judgment and ability to solve problems in a timely manner.
- Able to adhere to strict confidentiality standards.
- Demonstrated ability to work effectively both as a team member and independently.
- Demonstrated ability to work in a culturally diverse and inclusive environment.
