Wellness Navigator

About the role

The Wellness Navigator at Community Bridges, Inc. (CBI) plays a crucial role in removing personal and environmental obstacles to health care access. They work closely with the primary medical provider, the clinical team, and staff of other agencies involved in the patients' care to proactively manage patient care by providing case management, education, care coordination, systems navigation, advocacy, and support primarily in home and/or in the community.

Responsibilities

• Engage, educate, and offer support to individuals, their families, and caregivers to connect them to prevention, diagnosis, timely treatment, and follow-up.
• Work closely with the primary medical provider, the clinical team, and staff of other agencies involved in the patients' care to proactively manage patient care.
• Provide case management, education, care coordination, systems navigation, advocacy, and support primarily in home and/or in the community.

Requirements

• A high school diploma or GED.
• An associates degree (or higher) in a field related to behavioral health preferred.
• Education and/or experience in the field of health and nutrition preferred.
• A current AZ Driver’s License (valid and in good standing).
• A Clear 39-month Motor Vehicle Record (MVR).
• An Arizona Fingerprint Clearance card required.

Qualifications
