About The Role

We have a full time (37 hours) Enablement Coordinator position available in our Adult Social Care Enablement Service based in Forest. The Enablement Service supports and encourages all vulnerable adults who may have a disability and who also may have challenging home circumstances. The Enablement team aims to develop and promote daily living skills and greater control over people’s lives. The support work will take place in people’s homes, community, and in the Forest Drop In/s, which are based in the libraries. The Drop In/s are a free walk-in service that supports vulnerable adults who may need help with or advice about day-to-day life.

As an Enablement Coordinator, you will ensure the delivery of effective, person-centred approaches to individuals who are eligible for support, and focus on maximising their independence. You should be able to gather information accurately and concisely to support with preparing reports that reflect an individual’s needs. This role is ideal for someone with great communication skills and who can prioritise and manage their own time confidently. It is also important that the successful candidate continues to develop and maintain good working relationships with outside partnerships, in order to signpost individuals as required.

Responsibilities

• Deliver effective, person-centred support to individuals eligible for assistance.
• Maximise independence for vulnerable adults through skill development and encouragement.
• Gather and document accurate information to prepare reports reflecting individual needs.
• Maintain and develop strong working relationships with external partnerships for effective signposting.
• Work in people’s homes, community settings, and Forest Drop In/s (based in libraries).

Requirements

• NVQ Level 3 in Health and Social Care, Community Work, or equivalent qualification, or equivalent experience.
• Training related to Health and Social Care.
• Experience working with individuals who need support to achieve outcomes.
• Good interpersonal skills and strong verbal/written communication.
• Proficiency in IT, specifically Microsoft Office, and ability to learn in-house systems.
