About the role
The role involves providing Mental Health Screener functions as required by NJSA 30:4-27.1. It also includes performing psychosocial assessments, collaborating with medical center staff, developing treatment plans, and ensuring patient confidentiality.
Responsibilities
- Performs psychosocial assessments including collections of historical data, mental status assessments, and suicide risk assessments.
- Consults and collaborates with medical center staff to coordinate interdepartmental patient services.
- Develops comprehensive and realistic treatment plans based on patient’s needs.
- Provides appropriate short-term follow-up to ensure linkage and stabilization.
- Engages families, treatment providers, and other parties active in the well-being of the patient as appropriate whenever possible.
- Provides technical support, assistance, and education to patients, families, and other patient service providers as needed.
- Assumes the duties of charge person as required.
- Assumes the duties of mobile outreach on-call as required.
- Possesses thorough knowledge of all regulatory requirements for confidentiality.
- Demonstrates strict adherence to providing patient confidentiality as evidenced by documentation and behavior according to department policy.
- Maintains accurate and up to date patient records in accordance with regulatory requirements.
- Documents in clear, concise, and accurate manner in patient records according to department standards.
- Documents clinical assessment data in patient record as per department standard.
- Documents all collateral information obtained as per department standard.
- Documents changes in mental status in patient record.
- Documents all communications related to patient referrals, transfers, and admissions in the patient record as per department standard.
- Documents all telephone triage information obtained as per department standards.
- Ensures that screening document clearly details reasons for consensual admission or involuntary commitment.
Requirements
- Education: MSW in Social Work or MA in Counseling/Psychology or related field. LCSW or LPC preferred.
- Experience: One year experience in an acute mental health setting.
- Other Credentials: AHA BLS.
- Special Training: Permanent NJ State Mental Health Screening Certification obtained within one year of hire.
Qualifications
- Frequent physical demands include: Standing, Walking, Reaching forward, Keyboard use/repetitive motion.
- Occasional physical demands include: Sitting, Climbing (e.g., stairs or ladders), Carry objects, Push/Pull, Twisting, Bending, Reaching overhead, Squat/kneel/crawl, Wrist position deviation, Pinching/fine motor activities.
- Continuous physical demands include: Taste or Smell, Talk or Hear.
- Sensory Requirements include: Very Accurate 20/40, Near Vision, Very Accurate 20/40, Far Vision, Accurate Depth Perception, Accurate Hearing.
- Anticipated Occupational Exposure Risks Include the following: Bloodborne Pathogens, Chemical, Airborne Communicable Disease, Extreme Temperatures, Radiation, Uneven Surfaces or Elevations, Extreme Noise Levels, Dust/Particulate Matter.
