About the role
Do you want to utilize your case management skills to help individuals in our community? Community Action is looking for a committed Emergency Services Advocate to provide case management services to emergency services applicants. This individual will empower participants to reach their goals and help them access community resources.
Responsibilities
- Provide services for applicants by responding to all requests in a timely manner.
- Ensure all funding requirements are met for each applicant prior to approval.
- Conduct intake assessments on all applicants.
- Offer case management services to all funded applicants.
- Connect participants with appropriate agency and community resources.
- Aid participants in creating goal plans and follow through with progress.
- Maintain accurate and complete documentation and files on participants served.
Qualifications
- Associate’s Degree in human services or related field and/or equivalent work experience required.
- Case Management experience preferred.
- Strong verbal and written proficiencies of the English language required.
- Valid Nebraska Driver’s License and good driving record required.
- Sensitivity to the experiences, needs, and strengths of low-income and diverse populations, and individuals with disabilities.
- Knowledge of community resources.
