ED RN Case Manager - Inpatient Case Management CH

Location: CHA Cambridge Hospital – Inpatient Case Management. Full-time, unionized (MNA Cambridge) position.

About the Role

Under the general supervision of the Director of Care Management, the RN Case Manager provides clinically-based case management to support the delivery of effective and efficient patient care consistent with the Centers for Medicaid and Medicare Conditions of Participation. The RN Case Manager collaborates with the healthcare team to identify appropriate utilization of resources and ensure reimbursement, confirms medical necessity for admission and continued stay, and creates a post-acute care plan tailored to the patient’s needs and resources. This role facilitates transitions between inpatient and post-acute care teams.

Responsibilities

  • Conduct psychosocial assessments, evaluations, and referrals for adults and/or families with psychiatric illness, substance abuse, and medical illness in a hospital setting.
  • Cross-train across all areas of Care Management.
  • Collaborate with healthcare team members to optimize resource utilization and ensure reimbursement.
  • Use criteria to confirm medical necessity for admission and continued hospital stay.
  • Develop and facilitate post-acute care plans in partnership with patients, families, and care teams.

Requirements

  • Current or conditional Massachusetts RN licensure.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Certification in Case Management preferred.
  • American Red Cross (ARC) or American Heart Association (AHA) BLS certification required.
  • 3–5 years of medical/surgical RN experience or 3–5 years of RN Case Management experience.
  • Hospital Care Management experience strongly preferred.

Schedule

  • Full-time, day shift: 8:00 AM – 4:30 PM.
  • 8-hour shifts, rotating weekends as required.
  • Holidays per protocol; no on-call requirements.
