ED CARE NAVIGATOR

Responsibilities

• Performs PPS-related activities associated with care management, including identification of patient’s Health Home, Care Management agency and/or behavioral health providers; creating documentation; and adhering to workflows.
• Ensures that the patient is connected to primary care providers, and other community services, especially for those with multiple chronic conditions and/or uninsured.
• Captures and coordinates follow-up appointments with community providers.
• Safeguards discharge plan of care issues and ensures their resolution.
• Accesses electronic health record and PSYCKES and Healthify databases for care transitions/discharge summary purposes.
• Ensures and supports HIPPA compliance, including obtaining patient consents for information sharing among providers.
• Serves as a member of the Clinical Case Management team assigned to the Emergency Department, performing appropriate department functions, i.e. attending meetings and completing reports as required.
• Performs other duties as assigned.
• Serves as brand ambassador by upholding the WMCHealth Mission, Vision, and CARE values and promoting excellence in the patient experience, during every encounter.

Qualifications/Requirements

• Knowledge of healthcare industry and DSRIP preferred.
• Case management experience preferred, and/or experience in emergency dept. and/or working with highly vulnerable populations.
• Experience with Microsoft Office, HMIS training a plus.
• Strong communication skills.
