About the role

The Site Manager directs, organizes, and manages Serene Health’s Enhanced Care Management Program (ECM), coordinating physical health, behavioral health, and community-based long-term services and supports for eligible beneficiaries.

Supervisory Responsibilities

  • Oversees departments at various sites throughout the state.
  • Develops program goals and objectives, establishes staffing patterns, and organizes staff work.

Responsibilities

  • Leads a multidisciplinary team to provide a seamless continuum of care for high-need, high-risk populations.
  • Acts as liaison with other Serene Health departments, external referral resources, and Health Plan partners to maintain systems of care coordination.
  • Designs and implements systems of care coordination to enhance outcomes for high-risk patients.
  • Oversees the creation and implementation of Health Action Plans (HAP) for ECM patients and others requiring care, ensuring goals are met.
  • Develops, maintains, implements, and trains staff on ECM protocols, policies, and procedures.
  • Plans and conducts regular ECM staff meetings.
  • Ensures ongoing program compliance with ECM rules and regulations.
  • Oversees accurate documentation in Serene Health’s EHR system to meet program requirements, ensure high-quality care, and facilitate timely payment.
  • Extracts and reports on data from various systems to provide meaningful management information.
  • Gathers productivity and quality data from the ECM system and other sources for reporting.
  • Performs other duties as assigned.

Requirements

  • Bachelor’s degree from an accredited college or university or equivalent experience.
  • Minimum of 7 years of experience in a healthcare setting.
  • Experience managing multidisciplinary care teams.
  • Experience working with homeless, criminal justice, or other underserved populations.

Skills

  • Understanding of social determinants of health and their impact on patient well-being.
  • Culturally competent and able to work with a diverse population.
  • Bilingual (English/Spanish) preferred.
  • Strong proficiency with Microsoft Office applications (Word, Outlook) and internet applications.
  • Proficient in working with various electronic health record systems while maintaining data integrity and accuracy.
  • Excellent verbal, written, and interpersonal communication skills.
  • Strong problem-solving and organizational skills, with the ability to multitask and manage multiple priorities.
  • Able to communicate effectively at all levels of the organization, including leadership and high-level representatives of partner organizations.

Physical Requirements

  • Willingness to travel to different locations to conduct training sessions.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 10 pounds at times.

Pay

$76,000 USD - $105,000 USD
