Early Intervention Services Coordinator (Case Management) for Early Start Riverside South County

Under the supervision of the Program Manager - Early Start Program, the Early Intervention Services Coordinator is the first agency representative to families who have infants eligible for services under the U.S. Department of Education’s Individuals with Disabilities Education Act (IDEA), Part C. Responsibilities include coordinating assessment of infants, educating parents regarding developmental strategies, arranging for appropriate service intervention, making case dispositions, and performing liaison activities with referring professionals. Develop and implement Individual Family Service Plan (IFSP), perform case management activities, and complete the intake process. This position serves consumers in the Riverside South County area, including Lake Elsinore, Menifee, Murrieta, Temecula, Wildomar, and Romoland. Mileage reimbursement for business travel is provided.

Responsibilities

  • Assume daily case management responsibilities guided by the agency’s and program’s mission statement and core values.
  • Understand California’s regulations for the Early Start Program and Lanterman Act, differentiating eligibility and service provisions for each program.
  • Maintain required contact with consumers and their families, ensuring their rights and dignity are upheld.
  • Coordinate the annual review process, including completion of the Baby CDER, CDER, Annual Review, and IFSP.
  • Complete ongoing evaluation of consumer progress as identified in the IFSP.
  • Provide advocacy services, crisis intervention, and information and referral services to consumers and families.
  • Perform outreach and community liaison duties as assigned.
  • Arrange for placement of consumers in infant educational or training programs approved by the Interdisciplinary Team.
  • Perform assessments including First Look, Denver II, REEL III, Music 2 My Ears, M-Chat, and Day-C.
  • Instruct parents in developmental strategies and provide anticipatory guidance on child care, minor illnesses, nutrition, safety, immunizations, and medical follow-up.
  • Stay current on infant growth and development, parenting skills, developmental strategies, community resources, and information on specific disabilities.
  • Keep the Early Start Program Manager informed of caseload status.
  • Represent the regional center at case conferences and interagency meetings.
  • Complete administrative requirements, Purchase of Service forms, and Consumer File Record Documentation daily.
  • Complete requests for action (e.g., Eligibility Review, SIR closures, Quality Alerts) on a timely basis.
  • Ensure at least 95% of case-related paperwork is completed within designated time frames and transition children who have reached age three to appropriate education programs.
  • Complete TCM (Title 19) and Medi-Cal Waiver documentation accurately and on time.
  • Provide case management coverage upon request.
  • Maintain good attendance, punctuality, and compliance with agency policies and safety plans.
  • Contribute to team harmony by sharing expertise and maintaining effective interpersonal relationships.
  • Handle change well, demonstrating flexibility and adaptability.
  • Maintain equipment, work area, and safe driving record.
  • Utilize agency IT systems securely and follow protocols.
  • Organize travel efficiently and utilize bilingual skills as required.
  • Perform additional duties as assigned.

Requirements

  • Master’s Degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or a related field from an accredited college or university.
  • OR BA/BS degree from an accredited college or university and one year of experience in intellectual/developmental disabilities, social work, special education, early childhood development, or a related field.
  • Computer skills in Microsoft Office.
  • Ability to handle change, maintain confidentiality, and follow oral and written directions.
  • Strong verbal and written communication skills.
  • Full use of an automobile, valid driver’s license, and automobile liability insurance as prescribed by law, or ability to provide independent transportation. Must maintain a safe driving record.
  • Bilingual skills may be required.

Pay

Hourly range: $26.3130 - $40.8201

Schedule

Mileage reimbursement for business travel is paid out on a monthly basis.
