Duties
- Promote and adhere to the Supported Living Principles A Home of One's Own Choice and Self-Directed Relationships Community Membership Flexible, Tailored Services and Supports
- Assist in developing and implementing: Support service activities and Independent living training, Management plans Supported living assessments
- Carry out specific skills training plans for participants as instructed
- Assists participants in all personal care needs (toilet needs, feeding and hygiene)
- Aid participants with their mobility and communication needs
- Develop natural and community support
- Aid in medical appointments and health needs
- Maintain detailed documentation of services provided
- Participate in regular staff meetings
- Maintain appropriate communication with supervisor(s)
Physical Demands
- Stand, stretch, twist and reach with hands and arms for prolonged periods of time
- Lift 50 lbs or more
- Use hands to finger, to handle or to feel
- To talk and to hear others
- Regularly required to: Stand, walk, climb, balance and to drive
