About the Role
Provides care and support in a residential setting to individuals by assisting in the development and implementation of individual Support Plans, including spiritual life supports. Helps individuals achieve personal goals and desires, provides interaction and choices for meaningful daily activities, and supports independence in activities of daily living and personal care as needed.
Responsibilities
- Direct Care & Support (85%)
- Provides daily support, care, and companionship aligned with the individual Support Plan (ISP) with dignity and respect.
- Assists with personal care such as bathing, dressing, bathroom assistance, or housekeeping.
- Prepares and serves meals in accordance with individual diet plans.
- Administers medication and prescriptions as documented under the direction of a nurse.
- Schedules and transports individuals to medical appointments, outings, or errands as requested.
- Supports engaging activities to enhance quality of daily living and align with individuals’ personal goals.
- Maintains general event reporting (GER) and record-keeping in compliance with medical orders, state requirements, abuse reporting, court orders, and AbleLight’s internal procedures.
- Other Program Support (15%)
- Participates in the development and updating of individual support plans by providing insights into daily activities and interests.
- Purchases food and supplies as instructed.
- Maintains a clean, safe, and healthy environment in the residence.
- Ensures care and residence comply with safety protocols.
- Attends staff meetings and participates in continuing education.
Requirements
- Education: High School diploma or equivalent preferred, but not required unless directed by state regulations.
- Experience: 0 years required.
- Licenses/Certifications: Program-specific requirements based on state regulations; may require a Driver’s License with no negative Motor Vehicle Record findings.
