About the role

The Direct Support Professional (DSP) is responsible for providing direct oversight, guidance, and training to individuals receiving supports and services. This includes implementing goals outlined in their Staff Action Plan (SAP) and Individual Safeguards Plan or Individual Plan of Protection (IPOP).

Responsibilities

• Provide supports, assessments, and training to individuals according to their SAP and IPOP.
• Ensure necessary levels of supervision are met by adhering to the individual’s Safeguard Plan, IPOP, Behavior Support Plan (BSP), dietary guidelines, and other relevant plans.
• Accurately document services in the Electronic Health Record (EHR) or with paper notes, as required by the Agency, OPWDD, Medicaid, and other funders.
• Maintain accurate records by collecting, compiling, and evaluating data, and submitting it in a timely manner.
• Support the safety of individuals in various situations and assist with transportation as necessary.
• Work as part of a team to support positive behavioral strategies and assist individuals in building relationships within their community.
• Communicate health care concerns to supervisors or nurses as necessary and administer medications as per Agency policy and procedure.
• Participate in meetings and corporate compliance and quality assurance activities as required.

Qualifications

• High school diploma or equivalent.
• Valid NYS driver’s license with a safe driving record per agency policy.

$53,000.00 - $82,976.80 annually.

Schedule

5:00pm-9:00pm Friday, 3:00pm-11:00pm Saturday, 3:00pm-11:00pm Sunday.

20 hours per week.
