DME & Home Health Coordinator

About the Role

The DME and Home Health Coordinator acts as a liaison among patients, providers, insurance carriers, and community agencies to facilitate timely coordination of healthcare services, including DME, Home Health, and Specialty Services.

Responsibilities

• Manage prior authorizations and track referrals
• Coordinate patient care and advocate for patients with insurance
• Ensure accurate documentation in electronic health records
• Monitor authorization statuses and communicate effectively with stakeholders
• Assist patients with care navigation and education

Requirements

• High school diploma (required)
• Healthcare-related certifications and an associate degree (preferred)
• 1-3 years of experience in medical office settings, insurance authorization, or care coordination
• Strong communication skills and proficiency in computer applications
• Knowledge of insurance processes
• Background clearance, immunizations, and HIPAA compliance (mandatory)
• Organized, detail-oriented, and capable of managing a high-volume workload
