Discharge Specialist (Behavioral Care Center)

Based out of Nashville, MHC operates satellite offices in various communities.

Responsibilities

• Meets one-on-one with BCC C's to collaboratively develop a discharge plan addressing presenting problems and primary needs.
• Addresses mental health, physical health, substance use, housing, financial, legal, and social needs.
• Makes customer service a priority with internal and external customers, providing active listening and problem-solving.
• Facilitates communication regarding Consumer’s care with all relevant staff.
• Provides direct line services as needed on the unit, ensuring timely and appropriate documentation.
• Maintains and facilitates community and MHC interdepartmental relations through outreach and follow-up.
• Works collaboratively with other treatment staff and DCSO staff, participating in multi-disciplinary team meetings.
• Attends all mandatory in-services, continuing education, and annual health requirements.
• Reports all unethical activities to supervisor, Corporate Compliance Office, Values Hotline number, or Human Resource Director.
• Complies with all OSHA and safety policies, practices, and procedures, reporting all unsafe practices or accidents.
• Ensures compliance with relevant regulations concerning the privacy and security of consumer’s protected health information (PHI).
• Participates in proactive team efforts to achieve departmental and company goals.
• Performs other duties as assigned, demonstrating leadership through example and sharing of knowledge/skills.

Essential Skills And Experience

• Bachelor’s degree in health-related field such as counseling, psychology, or social work.
• Valid Tennessee Driver’s License.
• Acceptable Criminal Background Investigation.
• Personal Automobile Insurance with “F” endorsement.
