Discharge Specialist (Behavioral Care Center)
About the role

Founded in 1993, MHC serves individuals with severe and persistent mental illness and now also supports children and adolescents with severe emotional disorders across Middle and East Tennessee. Based out of Nashville, MHC operates satellite offices in various communities.
Responsibilities
- Meets one-on-one with BCC C's to collaboratively develop a discharge plan addressing presenting problems and primary needs.
- Addresses mental health, physical health, substance use, housing, financial, legal, and social needs.
- Makes customer service a priority with internal and external customers, providing active listening and problem-solving.
- Facilitates communication regarding Consumer’s care with all relevant staff.
- Provides direct line services as needed on the unit, ensuring timely and appropriate documentation.
- Maintains and facilitates community and MHC interdepartmental relations through outreach and follow-up.
- Works collaboratively with other treatment staff and DCSO staff, participating in multi-disciplinary team meetings.
- Attends all mandatory in-services, continuing education, and annual health requirements.
- Reports all unethical activities to supervisor, Corporate Compliance Office, Values Hotline number, or Human Resource Director.
- Complies with all OSHA and safety policies, practices, and procedures, reporting all unsafe practices or accidents.
- Ensures compliance with relevant regulations concerning the privacy and security of consumer’s protected health information (PHI).
- Participates in proactive team efforts to achieve departmental and company goals.
- Performs other duties as assigned, demonstrating leadership through example and sharing of knowledge/skills.
Essential Skills And Experience
- Bachelor’s degree in health-related field such as counseling, psychology, or social work.
- Valid Tennessee Driver’s License.
- Acceptable Criminal Background Investigation.
- Personal Automobile Insurance with “F” endorsement.
