About the role

The Discharge Planning Specialist (DPS) at St. Mary's Medical Center will assist the Care Facilitator and/or Social Worker with discharge planning responsibilities to promote timely and safe discharge services.

Responsibilities

• Assist the Care Facilitator and/or Social Worker with discharge planning responsibilities
• Facilitate and coordinate discharge services including home health, durable medical equipment (DME), rehabilitation placement, and other community resources or agencies
