About the role

Keck Medicine of USC is a community committed to enhancing healthcare through collaboration, growth, and well-being. This role supports the Case Management team in facilitating discharge planning and ensuring appropriate patient throughput.

Responsibilities

  • Works with the ambulatory care management nurse to manage a case load of up to 200 commercial outpatients.
  • Collaborates with the ambulatory care management nurse to develop and implement a plan of care for specific patient populations in the outpatient setting.
  • Assists the ambulatory care management nurse in coordinating post-acute services for patients, ensuring they receive the necessary care after discharge.
  • Participates in interdisciplinary meetings and team huddles to discuss patient care and discharge plans.
  • Communicates with providers, facilities, and patients regarding discharge status and requests.
  • Collects and documents data to support ongoing discharge planning and monitor patient progress.
  • Collaborates with social workers to address patient psychosocial needs and works with the care team to educate patients on self-management methods for chronic conditions.
  • Proposes alternative treatment options to optimize patient care and collaborates with the ambulatory care management nurse to ensure compliance with InterQual or MCG reviews.
  • Performs other duties as assigned.

Requirements

  • High school diploma or equivalent
  • 3 years of clinical experience
  • Experience with computer data entry
  • Typing speed of 40-55 words per minute
  • Good organizational and customer service skills

Preferred Qualifications

  • 2 years of experience in an HMO/IPA/Managed care setting
  • 1 year of care coordination experience within the last three years
  • Office skills

Benefits

The hourly rate range for this position is $29.00 - $45.20. Additional benefits may include health insurance, retirement plans, and paid time off.
