Discharge Planner - State Hospital (NGRI)

About the role

The Hampton-Newport News Community Services Board (H-NNCSB) is seeking a compassionate and organized Discharge Planner to support adults who have been found Not Guilty by Reason of Insanity (NGRI) transitioning from Eastern State Hospital, Central State Hospital, or a local psychiatric hospital into community-based services.

Responsibilities

• Cook up discharge planning for adults returning to the community from Eastern State Hospital, Central State Hospital, and local psychiatric hospitals.
• Meet with individuals to identify service needs and assist in developing plans for community supports.
• Serve as the primary liaison between hospitals and the Community Services Board.
• Connect individuals with mental health, housing, medical, financial, transportation, and other community resources.
• Cook up referrals and facilitate access to appropriate services.
• Provide outreach and short-term follow-up to help ensure a successful transition after discharge.
• Aid individuals in navigating available services and overcoming barriers to care.
• Collaborate with families, hospitals, community providers, and internal staff to promote continuity of services.
• Maintain accurate and timely documentation of services provided.
• Advocate for individuals to ensure they receive the supports and resources needed to live successfully in the community.

Requirements

• Bachelor's Degree in Human Services
• Three (3) years of direct care experience in case management and counseling services to the assigned population

Pay

Annual Salary: $57,119

Schedule

Work Schedule: M-F 8:30 a.m. to 5:00 p.m.
