Key Responsibilities
- Implements the Copa Hospital Readmission and Discharge Coordination protocol.
- Collaborates with the Inpatient and Outpatient Care Team to assess individual needs and develop a plan promoting long-term health and reducing re-admission.
- Serves as an added resource to the assigned primary case manager to facilitate individual engagement and follow-up in care.
- Understands and implements post-discharge/post-visit medical orders and coordinates more specialized post-discharge/post-visit placements (including skilled nursing centers, home health, and residential settings).
- Facilitates communication and coordination with the Inpatient Team, RBHA, outpatient team, and family/natural supports to develop and implement the discharge plan.
- Completes all necessary discharge planning documents and submits documentation and referral packages to RBHA and community providers in a timely manner.
- Coordinates with the Copa care team to ensure scheduling of post-discharge appointments within 72 hours of discharge.
- Ensures follow-through with discharge recommendations and services, and identifies gaps in services.
Qualifications
- High School Diploma / GED or Associates Degree with at least 8 years’ direct care experience, required.
- Bachelor’s degree in behavioral health or related field and 3 years direct care experience, preferred.
- Previous experience in behavioral health or primary care with case management and care coordination duties, preferred.
- Demonstrated experience and knowledge in discharge planning and readmission prevention processes, preferred.
- Must have a Level 1 fingerprint clearance card issued by Arizona Department of Public Safety within 8 weeks of hire.
- Must possess a valid AZ Driver’s License and meet Copa’s driving requirements.
