Discharge Planner, LVN
About the role
Facilitates, develops, and implements discharge planning in collaboration with the Case Manager, Social Worker, patient, family and treatment team.
Supports the patient, family, and members of the multidisciplinary healthcare team to ensure a safe and effective continuum of care to facilitate a smooth transition to the next level of care.

- Maintains current CA LVN license and BLS provider card.
- Has two years of acute care experience, preferably one year in acute hospital Discharge Planning and Case Management.
- Displays strong interpersonal skills, leadership, negotiation skills, good leadership talent, and knowledge of hospital operations.
- Shows general knowledge of the payer industry, resource management, reimbursement, and evidence-based clinical practice.
Requirements
Current CA LVN license.
Current BLS provider card.
Qualifications
Diploma from accredited LVN program.
