Essential Functions and Responsibilities
- Captures long-term and future patient care needs.
- Communicates activity status updates with clinical partners.
- Shares information and educates on community resources.
- Develops, coordinates, and communicates discharge plans.
- Documents assessments and overall discharge plans in the medical record.
- Collaborates with leadership to address discharge delays, barriers, and trends.
- Intervenes in cases of child/elder abuse/neglect and guardianship issues.
- Serves as a resource for treatment decisions surrounding end of life and Medical Power of Attorney.
- Facilitates meetings and comprehensive care planning with interdisciplinary teams.
- Delegates tasks to support team members.
- Utilizes post-acute care facilities for safe and effective discharge planning.
- Collaborates with contracted partners to facilitate post-acute facility placement.
- Conducts psychosocial assessments as needed for developing appropriate discharge plans.
- Performs other duties as assigned.
Qualifications
- Bachelor’s degree in Social Work required.
- One year experience in social work required.
- Experience with computer technology, especially Windows-based programs, e-mail, and Microsoft Word required.
- Experience in a healthcare field preferred.
- Certification in Case Management strongly desired.
Required Physical Requirements
- Constant use of arms and hands.
- Frequent standing, walking, and sitting.
- Occasional bending, stooping, and squatting.
- Able to lift up to 35 lbs.
- Able to push and pull up to 20 lbs.
- Auditory and visual skills, including color determination.

- Potential risk of exposure to radiation and toxic chemicals.
- Potential for exposure to bloodborne pathogens.
- Must be able to wear appropriate personal protective equipment.
Compensation
Pay Range: $30.90 - $43.26
