Responsibilities
- Coordinate the various activities of the Case Management Department under the direction of the assigned Case Manager/Social Worker.
- Aid in the development and implementation of discharge plans.
- Assist with case management referrals and authorizations.
- Maintain and coordinate Medicaid state submission forms for presentation to on-site reviewers and file them accordingly or as per facility’s procedure.
- Assist with Discharge Planning by interviewing patients/families and gathering data to develop a safe patient-focused Discharge Plan.
Qualifications
- Knowledge of Discharge Planning/Utilization Management/Case Management terminology and functions, in both managed care and non-managed care environments.
- Experience in basic to intermediate computer skills/knowledge.
- High School Diploma required.
- Current BCLS certificate required upon hire and maintain current preferred.
- Associates degree or LVN license preferred.
- Experience in the hospital setting preferred.
Pay
The current compensation range for this role is $23.00 to $30.08. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
