About the role

The Discharge Planner at Windsor Laurelwood Center for Behavioral Medicine is responsible for facilitating the transition of patients from the hospital setting to their home or community.

Responsibilities

• Develop discharge plans based on patient needs and medical conditions
• Coordinate with healthcare providers, family members, and community resources
• Ensure patients receive necessary follow-up care and medications
• Communicate with patients and families about post-discharge arrangements
• Monitor patient progress and adjust plans as needed

Requirements

• Bachelor's degree in a healthcare-related field
• At least 2 years of experience in healthcare administration or discharge planning
• Strong communication and organizational skills
• Ability to work independently and manage multiple tasks

Qualifications

• Knowledge of healthcare regulations and policies
• Experience with electronic health records (EHR)
• Proficiency in Microsoft Office Suite
