Discharge Planner

About the role

We are seeking passionate, patient-centric, and goal-oriented team members to join our dedicated behavioral health care team. Our mission is to provide compassionate and effective mental health and substance use treatment, focusing on reducing suicide rates, decreasing addiction-related deaths, and improving community productivity.

Responsibilities

• Actively involved in the discharge planning process
• Communicates with clients, families, and outside referral sources
• Demonstrates proactive communication with those involved with the patient’s treatment
• Provides resource information to patients and families as appropriate for future care needs
• Effectively communicates with UR on treatment and LOS issues
• Demonstrates appropriate crisis intervention and de-escalation skills
• Acts as a member of the team during crises, ensuring patient safety

Qualifications

• A high school degree or equivalent from an accredited college or university in social work, recreational therapy, or an appropriate therapeutic field preferred
• Combination of education and relevant work experience may be required
• Valid Driver’s License and a favorable Motor Vehicle Report and proof of insurance when using own vehicle
• CPR and CPI Certification within 30 days of employment
• Completion of additional age-specific training within 30 days of employment as required
• Must be at least 21 years of age
• Previous experience in a psychiatric health care facility, with direct experience working with chemical dependency, dual diagnosis, psychiatric, and geriatric patients preferred

Pay

$19.71 - 24.34 per hour. We expect those hired into this job to be paid within this hourly rate, but certain circumstances may allow us to pay outside of this range.
