Discharge Planner

About the role

Copper Springs, part of Lifepoint Behavioral Health, is a nationally recognized healthcare organization with a diverse network of facilities across the country. We provide compassionate behavioral health services, including crisis stabilization, inpatient care, and outpatient treatment for acute mental health and substance use disorders. Our mission is to decrease suicide rates, reduce addiction-related deaths, and improve overall community health through our psychosocial care approach.

Responsibilities

• Engage in active discharge planning processes
• Communicate effectively with clients, families, and external referral sources
• Provide necessary resources and information for future care needs
• Effectively communicate with UR regarding treatment and length of stay issues
• Demonstrate appropriate crisis intervention and de-escalation skills
• Assist in ensuring patient safety during crises

Requirements

• A high school diploma or equivalent, along with a degree from an accredited college or university in social work, recreational therapy, or a related therapeutic field (preferred)
• Relevant work experience in a psychiatric health care facility, with experience in chemical dependency, dual diagnosis, psychiatric, and geriatric patients (preferred)
• A valid driver's license and a favorable motor vehicle report with proof of insurance when using personal vehicles
• CPR and CPI certification within 30 days of employment
• Completion of additional age-specific training within 30 days of employment as required
• A minimum age of 21
• The ability to work flexible hours and overtime may be required
