Discharge Planner

As a Discharge Planner – Social Services, you will play a vital role in our mission to address the bio-psychosocial needs of our patients. This position involves performing social services functions and guiding patients in accessing treatment skills to achieve optimal emotional health.

Responsibilities

• Offering basic consultation, delivering specialized clinical social work services, crisis intervention, and conducting short-term and long-term family therapy.
• Communicating with referral sources within the first 24 hours of admission.
• Identifying needs of referral sources or professionals during the period of hospitalization.
• Overseeing the tasks of continued communication of treatment progress, discharge plans, and time of discharge to outside interested parties, as well as facilitating the transfer of required documents to the outside agents.
• Interfacing with the Business Development Department to assist with the determination of customer satisfaction and problem identification and solutions.
• Responsible for discharge plans including scheduling of appointments and communication with parents and significant family contacts.
• Aiding in chart audits and completion of medical records.
• Informing patients and families on discharge planning options based on diagnoses, prognoses, resources, and preferences related to home care services.

Qualifications

• Education: Bachelor's Degree from an accredited college or university, in Social Work or a clinical related mental health field preferred.
• Experience: Minimum of one (1) year experience in related disciplines with hospital experience strongly preferred.
• Licenses: N/A.

Additional Requirements

• CPI training within 30 days of employment and prior to assisting in a restraining procedure.
• Must be able to pass a competency exam for the area upon hire.
• May be required to work flexible hours and overtime.

Compensation

This is a full-time role and the expected compensation range is $15.00 - $19.00 hourly. We’re eager to engage with all qualified candidates, and consideration will be provided to experience and skill level.
