Role Overview
As a Discharge Lounge Nurse, you will deliver patient-centered care focused on seamless admissions and discharges in acute care settings.
Key Responsibilities
- Conduct nursing assessments, ensure proper admission procedures, and communicate with care teams regarding special needs or concerns.
- Complete documentation such as assessments, risk evaluations, medication reconciliation, and discharge paperwork.
- Educate patients, families, and caregivers about medical conditions, treatments, medications, and follow-up care, translating complex information effectively.
- Coordinate discharge processes, ensuring compliance with core measures and providing necessary educational materials and referrals.
- Prioritize workload in collaboration with nursing administration to facilitate smooth patient transitions.
Qualifications & Skills
- Registered Nurse (RN) with an Associate, Diploma, or Bachelor's Degree.
- Minimum two years of acute care nursing experience.
- Certification in Basic Cardiac Life Support (BLS) within 30 days and Advanced Cardiac Life Support (ACLS) within 6 months of employment.
- Strong assessment skills, attention to detail, and ability to prioritize tasks.
