About the role
Under the direction of the Supervisor of Care Continuum and collaboration with the bedside Case Management team, processes referrals for patients services to meet their discharge planning needs, assures a cost and quality effective discharge, and always promotes patients' rights and maximum benefits from health care services.
Requirements
- One year of experience in the medical field (hospital, home health, or nursing home).
- Preference for one year of discharge planning or processing referrals experience.
- Insurance coverage exposure is a plus.
Skills
- Excellent communication skills.
- Excellent interpersonal skills.
- Excellent organizational skills.
Qualifications
- Computer skills including Microsoft Office and electronic health records software.
Education
- Preferred: Bachelor's degree in a related field.
Benefits
Position is benefits eligible.
Schedule
- Part‑time schedule, Monday‑Friday.
- Six‑hour days: either 8 am‑2 pm or 10 am‑4 pm.
- Works 60 hours in a two‑week pay period.
