About the role
We are seeking an IRIS Consultant to support individuals in maintaining independence and self-direction within a social services setting.
Responsibilities
- Build trusting relationships with participants
- Assist in developing IRIS Support and Services Plans
- Maintain regular contact through monthly and quarterly visits
- Help participants understand IRIS eligibility requirements
- Connect with local agencies and ADRCs to broaden service options
- Document contacts and visits promptly within required systems
- Possibly conduct annual Long-Term Care Functional Screens
- Collaborate independently with a supportive ICA team
Qualifications and Skills
- Bachelor’s degree in social work, psychology, human services, counseling, nursing, or related field, or extensive social services experience
- Experience working with seniors or individuals with disabilities (minimum one year supervised or 4+ years of direct experience)
- Current IRIS Consultant status or DHS exemption preferred
- Multilingual skills a plus
- Strong commitment to participant autonomy and respect for diversity
- Proficiency with smartphones, tablets, and computer systems
- Knowledge of human behavior and community resources
- Valid driver’s license, reliable transportation, and satisfactory driving record

Schedule
