About the role

Join us as an IRIS Consultant to support individuals in living independently through the IRIS (Include, Respect, I Self-Direct) program. In this social services role, you will build trusting relationships with participants, develop personalized IRIS Support and Services Plans, and conduct regular contact and visits to ensure participant safety and autonomy. You will assist participants in understanding and maintaining IRIS eligibility, connect with local agencies to expand services, and document all interactions promptly. The role may include conducting annual Long-Term Care Functional Screens and collaborating with a supportive team to deliver participant-centered care.

Qualifications

  • A Bachelor's Degree in social work, psychology, human services, or related fields with at least one year of supervised experience working with seniors or individuals with disabilities
  • 4+ years of direct social services experience
  • Current IRIS Consultant in good standing
  • Bilingual skills are a plus

Strong commitment to participant autonomy, comfort with technology, knowledge of community resources, and respect for diverse populations.

Requirements

  • Valid driver's license, reliable transportation, and satisfactory driving record
