About the role

Join as an IRIS Consultant and empower individuals to live independently through the IRIS (Include, Respect, I Self-Direct) program. In this social services role, you will build trusting relationships with participants, develop IRIS Support and Services Plans, maintain regular contact and conduct visits to ensure participant well-being, assist with understanding and maintaining IRIS eligibility, connect participants with local agencies and resources, document all interactions accurately and promptly, and may conduct annual functional screens for long-term care. Work independently within a collaborative team environment.

Responsibilities

Requirements

• Valid driver’s license and reliable transportation
• Satisfactory driving record and auto insurance compliance

Qualifications

• Bachelor’s degree in social work, psychology, nursing, or related fields with experience working with seniors or individuals with disabilities, or extensive direct social services experience
• Bilingual abilities in languages such as Spanish, Hmong, Arabic, or Russian are a plus
• Strong commitment to participant autonomy and respectful service
• Proficiency with smartphones, tablets, and computer systems
• Knowledge of human behavior, social interactions, and community resources
• Willingness to serve diverse populations with empathy
