About the role

As an IRIS Consultant, you will support individuals in maintaining independence and self-direction within the IRIS program. Your responsibilities include building trusting relationships with participants, developing IRIS Support and Services Plans, and conducting monthly contacts and quarterly visits. You will assist participants in understanding and maintaining IRIS eligibility, connect with local agencies to expand service options, and document all interactions promptly. The role also involves conducting annual Long-Term Care Functional Screens and collaborating with a supportive team while working independently.

Qualifications & Skills

  • Bachelor's Degree in social work, psychology, human services, counseling, nursing, or related fields
  • At least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of direct social services experience
  • Current IRIS Consultants in good standing or bilingual skills are a plus
  • Strong commitment to participant autonomy
  • Comfortable with technology
  • Ability to serve diverse populations with empathy
