About the role
Join as an IRIS Consultant to support individuals in maintaining independence and navigating long-term care services.
Responsibilities
- Building trusting relationships with participants
- Developing IRIS Support and Services Plans
- Maintaining regular contact and conducting visits
- Helping participants understand and sustain IRIS eligibility
- Connecting with local agencies to expand service options
- Documenting all contacts and visits promptly
- Potentially conducting annual Long-Term Care Functional Screens
- Collaborating with a supportive team while working independently
Qualifications and Skills
- Bachelor’s degree in social work, psychology, human services, counseling, nursing, or related field, with at least one year of supervised experience working with seniors or people with disabilities, or 4+ years of relevant social services experience
- Current IRIS Consultant in good standing or DHS exemption preferred
- Multilingual skills are a plus
- Strong commitment to participant autonomy and respectful service
- Proficiency with smartphones, tablets, and computer systems
- Knowledge of human behavior, social interactions, and community resources
- Willingness to serve diverse populations with empathy

- Valid driver’s license and reliable transportation
- Good driving record and ability to meet auto insurance requirements
