About the role

Join as an IRIS Consultant to support individuals in maintaining independence and navigating long-term care services.

Responsibilities

  • Building trusting relationships with participants
  • Developing IRIS Support and Services Plans
  • Maintaining regular contact and conducting visits
  • Helping participants understand and sustain IRIS eligibility
  • Connecting with local agencies to expand service options
  • Documenting all contacts and visits promptly
  • Potentially conducting annual Long-Term Care Functional Screens
  • Collaborating with a supportive team while working independently

Qualifications and Skills

  • Bachelor’s degree in social work, psychology, human services, counseling, nursing, or related field, with at least one year of supervised experience working with seniors or people with disabilities, or 4+ years of relevant social services experience
  • Current IRIS Consultant in good standing or DHS exemption preferred
  • Multilingual skills are a plus
  • Strong commitment to participant autonomy and respectful service
  • Proficiency with smartphones, tablets, and computer systems
  • Knowledge of human behavior, social interactions, and community resources
  • Willingness to serve diverse populations with empathy

Requirements

  • Valid driver’s license and reliable transportation
  • Good driving record and ability to meet auto insurance requirements
