About the role

Supporting older adults in maintaining independence and quality of life.

Responsibilities

• Provide comprehensive care services to participants.
• Collaborate with healthcare providers and community resources.
• Ensure compliance with regulatory standards and PACE guidelines.
• Engage with participants and their families to understand needs and preferences.

Requirements

• Bachelor’s degree in a related field (e.g., nursing, social work).
• Experience working with older adults.
• Knowledge of PACE program requirements and regulations.
• Excellent communication and interpersonal skills.

Qualifications

• Valid driver’s license and reliable transportation.
• Ability to work flexible hours, including evenings and weekends.

Skills

• Strong organizational and time management skills.
• Proficiency in Microsoft Office Suite.
• Ability to work independently and as part of a team.

Pay

Salary range: $50,000 - $65,000 annually.

Schedule

Variable schedule based on participant needs, typically includes evenings and weekends.
