Director of Social Work

Job Summary

Executive Search Network/Platinum Staffing is seeking a Director of Social Work for a skilled nursing, long-term care facility in Suffolk County, New York.

Job Responsibilities

• Aid new residents and their families during the initial adjustment and ongoing transition to institutional living.
• Provide information about residents' rights and responsibilities.
• Interview residents and families to gather social history and participate in discharge planning.
• Develop and implement social care plans and resident assessments.
• Interpret social, psychological, and emotional needs of the resident care team and involve residents/facility in planning objectives and goals.
• Refer residents and families to appropriate social services agencies and arrange transportation to other facilities.

Qualifications/Requirements

• Experience as a Social Worker in a Nursing Home.
• Strong written and verbal communication skills.
• Excellent communication skills.

Requirements
