About the role

Vista Grande Rehabilitation & Healthcare Center is seeking a compassionate and organized Social Services Director to oversee and coordinate the facility's Social Services Department. The ideal candidate will provide input and documentation for the MDS process, advocate for residents' emotional, social, and psychosocial needs, and participate in interdisciplinary care conferences.

Responsibilities

  • Oversee and coordinate the facility's Social Services Department
  • Complete psychosocial assessments, care plans, and required documentation
  • Provide input and documentation for the MDS process as required
  • Advocate for residents' emotional, social, and psychosocial needs
  • Build trusting relationships with residents and their families
  • Participate in interdisciplinary care conferences and collaborate with nursing, therapy, dietary, and other departments
  • Aid residents and families with discharge planning, transfers, and connections to community resources
  • Support residents and families through changes in condition and care transitions
  • Address resident concerns and assist with problem-solving and conflict resolution
  • Develop relationships with community organizations and local resources
  • Maintain compliance with state and federal regulations related to social services
  • Support resident quality of life, dignity, independence, and personal choice

Requirements

  • Bachelor's degree in social work, Human Services, Psychology, Sociology, or a related field preferred
  • Certification in Colorado social work licensure, such as LSW or LCSW, preferred but not required
  • Previous experience in social services, case management, behavioral health, healthcare, senior services, or a related setting preferred
  • Skilled nursing or long-term care experience is a plus but not required
  • Knowledge of MDS, care planning, and long-term care regulations is preferred
  • Strong communication, organization, and problem-solving skills
  • Ability to build positive relationships with residents, families, staff, and community partners
  • Compassionate, dependable, and committed to resident-centered care

Qualifications

  • Experience in a healthcare or social services setting
  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Knowledge of relevant laws and regulations
