About the role
Provide leadership, oversight, and direction for the Social Services Department, including developing, implementing, and evaluating departmental policies, procedures, and staff performance.
Responsibilities
- Recruit, interview, hire, onboard, and supervise Social Services staff in collaboration with the Human Resources Department.
- Coordinate resident admissions by completing psychosocial assessments, required documentation, resident and family orientation, and addressing adjustment concerns.
- Manage discharge planning by coordinating interdisciplinary meetings, arranging home health services, durable medical equipment, and community resources to support successful transitions.
- Facilitate care plan meetings, family conferences, Resident Council meetings (as requested), and provide ongoing support and advocacy for residents and their families.
- Complete timely and accurate social services documentation, including MDS assessments, BIMS, PHQ-2/PHQ-9 screenings, and other required psychosocial assessments.
- Investigate resident concerns, grievances, and allegations of abuse or neglect in collaboration with the Administrator and interdisciplinary team, maintaining confidentiality and regulatory compliance.
- Participate in chart audits, quality assurance initiatives, interdisciplinary meetings, and state and federal surveys to ensure high-quality care and regulatory compliance.
- Protect resident rights and collaborate with the Administrator to develop and implement Plans of Correction for social services-related survey deficiencies.
- Participate in the Manager-on-Duty rotation and perform other duties as assigned by the Administrator to support facility operations and resident-centered care.
Qualifications
- Master's Degree.
- Maryland State Licensure as a Licensed Certified Social Worker (LCSW) or Licensed Certified Social Worker - Clinical (LCSW-C).
- Experience in health related facility is recommended.
- Knowledge of nursing practices and procedures, as well as laws, regulations and guidelines that pertain to long term care, or must be willing to acquire.
- Must possess leadership skills and be detail-oriented.
- Good communication and interpersonal skills essential.
- Computer literacy using Windows OS, Microsoft Office Suite, Google Suite.
- Knowledge of Point Click Care or other EHR.
