Director of Social Services

Social Services Responsibilities

• Direct and oversee the daily operations of the Social Services Department
• Complete psychosocial assessments and develop individualized care plans
• Cook up resident admissions, transfers, and discharge planning
• Provide counseling, emotional support, and crisis intervention for residents and families
• Ensure compliance with federal, state, and local regulations related to social services
• Collaborate with the interdisciplinary team to promote quality resident care
• Aid residents and families with advance directives, community resources, and discharge arrangements
• Address resident concerns, advocate for resident rights, and assist with conflict resolution
• Maintain accurate and timely documentation within the electronic medical record
• Participate in care plan meetings, quality improvement initiatives, and regulatory surveys
• Supervise and mentor Social Services staff as applicable

Social Services Qualifications

• Bachelor's degree in Social Work (BSW) required; Master's degree in Social Work (MSW) preferred
• Current Pennsylvania Social Work licensure, if applicable, preferred
• Previous experience in a Skilled Nursing Facility or Long-Term Care setting required
• Prior supervisory or leadership experience preferred
• Strong knowledge of admissions, discharge planning, resident rights, and long-term care regulations
• Excellent communication, organizational, and interpersonal skills
