Qualifications
- Current LSW or CSW required
- Prior supervisory or management experience preferred
- Current AHA BLS CPR Certification recommended
- Strong communication, organizational, and documentation skills
- Computer proficiency required
- Ability to work independently and as part of an interdisciplinary team
Responsibilities
- Lead and oversee the Social Services Department at Complete Care at Brick
- Develop and implement social service programs, policies, and goals
- Advocate for residents' psychosocial, emotional, and behavioral health needs
- Participate in interdisciplinary care plan meetings and collaborate with the clinical team
- Maintain accurate, timely, and compliant documentation
- Educate residents and families regarding Advance Directives and available resources
- Coordinate admissions, transitions, and discharge planning to ensure positive outcomes
- Provide guidance and support to social services staff
- Assist residents and families with community resources and support services
- Perform additional duties as assigned by the Administrator
