You will lead our Social Services Department, ensuring the emotional, social, and psychological well-being of our residents in a supportive and collaborative environment.
Responsibilities
- Develop and implement comprehensive social services programs that address the diverse needs of our residents.
- Provide leadership to the Social Services team, ensuring the delivery of high-quality, person-centered care.
- Collaborate with interdisciplinary teams to create individualized care plans that enhance the overall well-being of residents.
- Act as a liaison between residents, families, and community resources to support a seamless transition into our facility and assist with discharge planning.
- Advocate for residents' rights, ensuring they receive the highest standard of care and maintaining their dignity and independence.
Requirements
- Master’s degree in social work or related field.
- Licensed Social Worker (LSW) or Clinical Social Worker (LCSW) preferred.
- Minimum of 4 years in social services, with at least 2 of those years in a leadership role in a long-term care setting.
- Strong understanding of the aging process, psychosocial needs of the elderly, and the impact of chronic illness.
- Ability to collaborate effectively with a diverse team and build positive relationships with residents and their families.
