Responsibilities

• Provide professional social work services to our residents, including assessments, counseling, and care planning
• Develop and implement individualized care plans to address the social, emotional, and psychological needs of our residents
• Collaborate with other healthcare professionals to ensure comprehensive care and support
• Facilitate family conferences and support groups to promote communication and empowerment
• Maintain accurate and timely records of services provided
• Participate in quality improvement initiatives to ensure the highest level of care and service

Requirements

• Licensure as a Licensed Social Worker (LSW) in the State of New Jersey
• 2 years of experience as a Social Worker
• Strong communication and interpersonal skills
• Commitment to providing exceptional care and service to our residents
• Master's degree in Social Work (MSW) from an accredited institution
