About the role
We provide skilled nursing care to people recently discharged from the hospital, needing therapy & to those needing long-term care.
Responsibilities
- Supervises Staff: Manages, hires, trains, and schedules social work staff and interns.
- Administrative: Establishes departmental policies, procedures, and quality improvement standards & oversees the social services department budget and resource allocation.
- Enforces Regulations: Ensures the facility complies with federal (CMS), state, and local social service mandates & Serves as the primary department contact during state health inspections and surveys.
- Maintains Documentation: Maintains & audits medical charts to ensure psychosocial documentation meets strict legal standards.
- Collaboration: Participates in care planning and other meetings, including Quality Assurance and Performance Improvement (QAPI).
- Care & Mediation: Steps in resolving severe, complex family grievances or resident crises. Provides supportive care as needed to supplement social work needs.
- Community Liaison: Builds relationships with local hospitals, mental health organizations, and community resources to streamline admissions and discharges.
Requirements
- Prior Long Term Care Experience
Qualifications
- Licensed to work within the State of Connecticut as a LSW
- Able to pass background check, drug screen, professional references check & physical evaluation
- High level understanding of leadership, customer service and clinical competency
- Prior experience as a Director of Social Services/Social Work Director
