Director of Social Services

About the role

Golden Age Manor, owned and operated by Polk County, is seeking a compassionate Director of Social Services to join our team. This full-time, Monday–Friday position (8:00 a.m.–4:00 p.m.) is ideal for a licensed social worker with a genuine passion for supporting older adults and individuals in need.

Responsibilities

• Develops and implements social services policies and procedures.
• Coordinates intake admissions and discharge procedures.
• Assists in counseling and social services to residents and their families.
• Completes or assists in completing various benefit forms.
• Provides information and assistance to families regarding resident care and finances.
• Monitors and updates resident care levels and conditions.
• Develops and implements interdisciplinary care plans.
• Investigates and resolves resident complaints and grievances.
• Ensures compliance with state and federal regulations.
• Supports residents and their families through transitions and adjustments to long-term care facilities.
• Communicates with medical providers and psychiatrists to meet individual needs.
• Participates in the development of the facility's marketing program.
• Serves as a liaison between the nursing home and other agencies.
• Conducts personnel in-service meetings on resident rights and issues.

Qualifications

• Bachelor’s degree in Social Work or a closely related Human Services program.
• One year of experience as a Social Worker.
• Valid Wisconsin Social Work License.
• Successfully passed criminal and caregiver background check.
• Nursing Home experience preferred.
