Director of Health & Wellness

About the Role

Provide strategic leadership for the wellness department in the community by collaborating with each department to coordinate care and services that promote residents' quality of life. Direct management of the care team and medication providers, as applicable by State/Province law, to ensure the health and wellness of residents.

Responsibilities

• Provide care through the direct application of the care process: perform and document resident assessments, complete Individualized Service Plans, provide or delegate hands-on clinical care as indicated by the plan of care, and evaluate resident outcomes.
• Oversee the community medication management program to promote resident safety in the medication use process.
• Collaborate with physicians, pharmacists, and other ancillary partners to coordinate care and services for residents.
• Provide training and education to the care team and perform skill evaluations to develop and promote clinical capabilities.
• Monitor and assess care changes and needs to identify risk; lead care quality and process improvement initiatives to mitigate risk and improve resident care outcomes.
• Recruit, hire, and train care team members, managing performance and engagement.
• Complete direct report team member staffing and scheduling according to operational and budgetary guidelines.
• Support regulatory compliance for community clinical and care items.
• Manage resident event response and care coordination.

Requirements

• Active LPN or RN License.
• 1+ year of experience in home health, assisted living, memory support, or long-term care preferred.
• 1+ years of nursing management experience, including hiring, coaching, performance management, scheduling, and daily operations supervision.
• Academic degree in a related field may be accepted as allowed by state regulations.
• Strong communication, computer, community relations, and personnel skills.
• Commitment to a holistic approach to health, safety, and overall well-being.

Qualifications

Clinical health experience is a plus.
