About the role
The Director of Field Clinical Operations, RN position at ChoiceCare Home Health is a 100% travel role across the Chicagoland area, focusing on home health services. The role requires a strong background in nursing, with a focus on leadership and collaboration.
Responsibilities
- Oversee the collaborative process among members of the Interdisciplinary Team (IDT).
- Ensure home health services are available 24/7 and act as the agency Administrator when needed.
- Manage IDT members, including scheduling, productivity, mentoring, performance reviews, and support.
- Collaborate on QAPI activities and provide input into Performance Improvement.
- Uphold a philosophy of integrity and commitment; communicate proactively about schedule changes.
- Participate in patient care as needed, including admissions, recertification visits, routine visits, and discharges.
- Manage patient caseloads to support census changes.
- Develop and deliver comprehensive training programs for new team members, covering clinical protocols, operational procedures, and EMR system usage.
- Provide operational support, clinical direction, and leadership to address program-level challenges.
- Ensure accurate and timely documentation in patient clinical records.
- Analyze and report on quality metrics, including HHCAHPS and Home Health Quality outcomes.
- Participate in after-hours shifts and on-call responsibilities as needed.
De Novo Expansion Responsibilities
- Ensure compliance with state regulatory requirements for nursing services in new and existing locations.
- Lead the strategic planning and execution of new home health locations.
- Oversee the logistical aspects of establishing new locations, including facility setup and equipment procurement.
- Recruit, hire, and train dedicated teams for each new location.
- Provide ongoing leadership and mentorship to new teams, fostering a positive and collaborative work environment.
- Collaborate with regulatory bodies to obtain necessary licenses and certifications.
- Conduct regular gap analysis and implement solutions for improvement.
Qualifications
- Education: RN, Required; bachelor’s degree in nursing preferred but not required.
- Experience: Minimum of 5 years of nursing experience, with 3 years in home health required; 5 years of management experience, with 2 years in hospice or home care required.
- Skills: Proficient in Homecare Homebase (HCHB) EMR for home health, strong mathematical and language skills, and the ability to articulate and embrace integrated healthcare at home philosophy.
- Certifications: Active and unencumbered Registered Nurse or Physical Therapist license or COMPACT license preferred.
Physical Demands And Work Environment
- Regular standing, walking, and manual dexterity are fundamental.
- Ability to lift and move objects up to 50 pounds.
- Exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected.
- Routine use of standard medical equipment and tools associated with clinical care.
