Director of Clinical Care

About the Role

The Director of Clinical Care, under the direction of the Chief Nursing Officer (CNO), is responsible for desired clinical and financial outcomes, collaborative practice, and the allocation of human and material resources. This role utilizes the interdisciplinary team model in hospice and palliative care to ensure improving quality of care, balancing quality with financial stewardship.

Responsibilities

• Utilize the interdisciplinary model to promote cost-effective, quality care within the hospice environment.
• Promote and model collaborative interdisciplinary practice processes for coordinated continuity of care and optimal outcomes.
• Evaluate the financial and clinical benefit of the Care Plan against patient outcomes.
• Practice in accordance with the mission, philosophy, policies, and ethics of Alive Hospice, Inc., NHPCO guidelines, and Joint Commission standards.
• Serve as a liaison, educator, and resource for hospice in professional and community settings.
• Support management in achieving agency goals through project work and team building.
• Provide clinical and administrative supervision to line staff, including recruiting, selecting, training, counseling, scheduling, and assignments.
• Manage systems, processes, staffing, budget, and outcomes in collaboration with the CNO.
• Supervise and allocate clinical staff based on agency needs.
• Collaborate with staff from other departments as appropriate.
• Manage medications, stock, and supplies as assigned.
• Coordinate admissions to home care teams and/or Inpatient Units.
• Perform other duties as assigned.

Requirements

• RN or MSN with a current Tennessee license (or compact RN license).
• Four-year college degree (BSN preferred).
• Three years of applicable experience in a related field (hospice experience preferred).
• Current and valid Tennessee driver’s license and automobile insurance as required by Tennessee State Law.

Preferred Qualifications

• Master’s Degree.
• CHPN or CHPCA certification.
• Three years of management experience.
• Hospice and Palliative Care certification.
