Director of Care Management, Nurse Practitioner (PICU Experience Required)

About the role

The Director of Care Management (DCM) oversees the inpatient RN Case Management team at Blythedale Children’s Hospital. The DCM ensures effective patient throughput, timely admissions, and a positive patient and family experience during their stay. Key responsibilities include developing processes to manage hospital referrals, creating an environment for continuous quality improvement, and supporting the use of best practices for process improvement.

Responsibilities

• Develops processes to monitor hospital referrals ensuring effective management of admission and discharges.
• Ensures effective patient throughput, timely admission, and a positive patient and family experience during their stay.
• Creates an environment for continuous quality improvement and supports the use of best practices to make recommendations for process improvement.
• Creates a feedback loop to develop systems for patient admission and discharge that ensure timely and accurate transfer of information and provide specific data to understand and analyze utilization of hospital resources to improve efficiency, ensure quality care, and support accurate hospital billing to improve hospital financial outcomes.
• Analyzes all incoming referrals for appropriateness, assesses clinical, psychosocial, and rehabilitation needs, and understands discharge disposition.
• Directs, plans, implements, and evaluates care coordination of patient care across the continuum of care; collaborates with other hospital directors, establishes goals and objectives for the department.
• Collaborates with physician leadership to develop strategies, formulate plans, and implement interventions to improve the safe admission and discharge of patients.
• Develops and implements policies and procedures to create clarity for internal and external providers regarding clinical and rehabilitation services.
• Collaborates with Medical Director and Nurse Educators to develop strategies to mitigate costs for transporting patients through the development of new skills and clinical programs on the inpatient unit.
• Analyzes patient and resident data to understand referral patterns, length of stay trends, and barriers to discharge in order to create strategies for expansion or modification of diagnosis specific services at Blythedale.
• Collaborates with Patient Access and Social Work teams to support the financial and psychosocial needs of the patients and ensure that all referrals are reviewed for appropriateness prior to admission.
• Performs other functions at the discretion of the Medical Director and/or Medical Office Leadership.

Requirements

• Experience in a pediatric setting required, PICU experience required.
• Experience with Meditech or another EMR is helpful.
• Experience in Quality Improvement project management preferred.
• New York State licensure as a Nurse Practitioner is required.
• A Master of Science in Nursing is required.
• PRI and Screen Certification helpful.

Pay

$160,000.00 - $180,000.00 /Yr.

Schedule

Generally, Monday-Friday, 8am-5pm.
