Director of Care Management, Nurse Practitioner (Pediatric/PICU)

Key Responsibilities

  • Lead, mentor, and manage the inpatient RN Case Management team.
  • Develop and implement departmental goals, policies, and best practices.
  • Oversee patient admissions, referrals, care coordination, and discharge planning.
  • Evaluate incoming referrals to determine clinical appropriateness and rehabilitation needs.
  • Collaborate with physicians, nursing leadership, social work, patient access, and other interdisciplinary teams to optimize patient flow.
  • Analyze referral trends, patient length of stay, utilization data, and discharge barriers to enhance patient care and organizational performance.
  • Build and maintain relationships with referring healthcare organizations and providers.
  • Aid in developing strategies to improve patient throughput while maintaining high standards of clinical care.
  • Participate in departmental planning, performance improvement initiatives, and leadership meetings.
  • Ensure compliance with regulatory standards and organizational policies.

Required Qualifications

  • Master of Science in Nursing (MSN).
  • Active Nurse Practitioner license (or ability to obtain licensure in the hiring state before employment).
  • Previous experience working in a pediatric healthcare setting.
  • Prior Pediatric Intensive Care Unit (PICU) experience.
  • Strong leadership, communication, and interdisciplinary collaboration skills.
  • Experience coordinating patient care across the continuum.

Preferred Qualifications

  • Previous leadership or management experience in Case Management or Care Coordination.
  • Experience with electronic medical record (EMR) systems.
  • Quality Improvement or Performance Improvement project experience.
  • PRI and Screen Certification is a plus.
